Terms of The East Devon Cystic Fibrosis Quality of Life Fund
The Quality of Life Fund is set up to fund raise and to help people with Cystic Fibrosis who live in East Devon by providing help by way of a Grant for a piece of medical equipment, physiotherapy equipment etc. which they feel would greatly enhance their quality of life.
The Applicant has to have Cystic Fibrosis or The Applicant must be applying on behalf of a person who has Cystic Fibrosis.
The Applicant must live in the East Devon Area.

The Specialist Nurse/Doctor to be written to with consent of the Applicant to confirm that The Applicant is under the care of the Doctor and that the piece of equipment for which the applicant is applying for the grant is needed and will greatly improve their ‘quality of life’.
Meetings will be held every two/three months, where applications will be considered and grants awarded. The decisions of the grant application will be made by the Trustees of the Fund.
Payment to be made by way of a cheque made out to the supplier of the item applied for. Only in exceptional circumstances if the item applied for has already been purchased, the cheque will be made out to the applicant only if the original proof of purchase has been sent in. These items purchased need to have been made in the last two months.
The Quality of Life Fund will continue to fundraise and to give half the monies raised to the Cystic Fibrosis Trust, by way of payment once yearly, by 31st March, half to the Quality of Life Fund.

All applicants to be sent details of any future Fundraising Events and to try to help out, as and if/when possible, even by way of putting up a poster, drumming up support, etc.
